For some odd reason, some newly converted mp4 videos get the 200 stream error and then others don't get it which are all converted from the same folder. What is causing some videos to get the error? and how to fix it?

I think this is a hotlinking issue...but could be wrong

Ummm, it's seems like the 200 stream error only occurs if i convert a FLV to MP4.
Fix... I notice this script is more picky or should i say more accurate. "Upload from FTP" if you don't select the "convert all videos" then it would act like it convert to MP4 but in reality it doesn't, it just show MP4 and you will get 200 stream error. Odd thing is for AVI, WMV you don't have to chose "convert all". Guess my next test is to see what happen with a FLV when uploaded from the front end.
EDIT: Front end works perfect.
Conclusion, make sure you select "convert all videos" or you could end up with 200 stream error from empty containers.
