I keep getting an error message when I load a video the media could not be loaded because the server or network failed or because the format is not supported. Not sure how to fix this issue.

The only reason for this to happen is if you double clicked the add button. You can delete the duplicates from Admin -> Video -> Tools -> Duplicate Videos.
