2. a log file of moderator actions may be helpful too to check what moderators did if needed (well dun real know yet what permissions moderators have lol) .
3. guest / users who flag a video should be forced to add a comment too.

1. Hmm...can you please explain for what exactly will use this? (you are the admin and even if you have more admins, you know those people).
2. Currently the moderator system is not implemented correctly and it will be updated in the 1.2 version (1.1 is for development).
3. A option for this could be added, but not forced by default.

To 1. It´s for security reasons. If you see for example robert , mmmmmm but do not have admins with these names or you even see your own name there several times it may be a hack attack. Forgot one important point in the above post - the ip

Thats not really needed :-) You can just password protect your admin via .htaccess and htpasswd, if you want more security.
