Premium module question.

hi, i installed premium module on my site but when somthing with .htaccess is wrong becouse there is no any ulrs.
when i click pay. or click to upgrade i get blank page with url: http://www.dadsporno.com/premium/upgrade/

please help i can't find documentation

I just checked and i get the login page. Thats how its supposed to work.

o sorry, take this user: Dady / xpcom111 (pass)

and tray to order "Premium 1 Month Access" (last one) via credit card. and get blank page
