Fixed on your site, however on my box there are no duplicates and i didnt have to add DISTINCT to the mysql query.

Edit modules/video/components/tag.php and find:
$sql = "SELECT v.video_id, v.title, v.slug, v.rating, v.rated_by, v.duration, v.thumb,replace with:
$sql = "SELECT DISTINCT v.video_id, v.title, v.slug, v.rating, v.rated_by, v.duration, v.thumb,\
I think the bug must be somewhere else. Like video edit in admin (or frontend) or some mass edit/bulk edit feature.

Find. Added Admin -> Video -> Tools -> Tags Rebuild (just in case any duplicate tags are missing, the tags will be cleaned here). Also check/fixed each video edit to check for duplicate tags.
