when attempting to upload a video either as an admin or as a user, immediately after upload the page refreshes with a 404 Page within the template stating - We're sorry, the page you requested cannot be found! (it's within the ASP template).
any suggestions on a cause and a fix?
I've tested with MPG and FLV, same result. 404 immediately after upload.

Like i told you yesterday via email, mod_security is the problem. You will need to disable mod_security to make this work, or at least find and change the rules that prevent the upload from working. I will test with mod_security disabled today.
And I sent you a reply concerning that and you never replied. You can't just disable mod_security on the whole platform due to my other sites. If you read the email that I sent you earlier, which again, you never replied, you'll see what I asked..

Ok, so my problem may not be this problem that you are referencing. I had an issue on import where after completion (it did import the videos, thumbs, etc) it went to a 404 page in the non-admin template. It was not an issue for me so I never posted. As far as my site goes, it is in holding as I am getting everything perfect before I launch.

a lot of people just put solved in the post but they never put what they did to solve it. So, below is the resolution to this issue. It's good posting standards to be sure to include what was actually done to solve a problem and not just put "i fixed it... "
SOLVED - it was the mod_security. You need to either not run mod_security on your server which is NOT suggested, or add a rule to whitelist or exclude the "offending" file.
To find out the proper syntax we need to figure out the error that is
occurring in the apache main error log.
Once you find that out we need to locate the proper whitelist or exclude file
for mod_security.
Below is an example but the exact syntax and file locations may
vary.
apache error log:
/usr/local/apache/conf/logs/error_log
sample error would look like: ModSecurity: Access denied with code 406 (phase
2). Pattern match "^Shockwave Flash" at REQUEST_HEADERS:User-Agent. [file
"/usr/local/apache/conf/modsec2.user.conf"] [line "203"] [hostname
"www.domainname.com"] [uri "/wp-admin/php/upload.php"] [unique_id
"byevW0Wnsx4AAGyHtncAAAA-"]
mod_security whitelist file:
/usr/local/apache/conf/modsec2/whitelist.conf
syntax to whitelist a domain would be something like:
SecRule SERVER_NAME "domain.com" phase:1,nolog,allow,ctl:ruleEngine=Off
We don't actually recommend whitelisting whole domains as whitelisting can be
setup per url.

White mod_security might bring some security to your site, it slows down the requests and it breaks a lot of servers. Also as a user you cannot disable it (you were able to disable mod_security 1 via .htaccess, but not mod_security 2).
I'm going to check your server again today and make sure that lighttpd also works.

The wordpress listed above is just an example for syntax. Not what I actually did..... I whitelisted the the whole domain in sec2.

Sometimes I too have the same type of problem. I am thinking if it is due to internet connectivity problem or something else
